What this policy covers
This policy explains how drtollesson.com.au, this website, handles personal information submitted through it. It covers the website only.
Health information collected in the course of a consultation, and the clinical records arising from it, are held by the practice, Brain and Spine Surgery Queensland, and are governed by the practice's own privacy arrangements and by Australian law rather than by this page.
The law that applies
Private health service providers in Australia are bound by the Privacy Act 1988 (Cth) and the Australian Privacy Principles. Health information is classified as sensitive information under that Act and attracts a higher level of protection than ordinary personal information.
WHAT IS COLLECTED
What this website collects, and what it does not
The personal information this website collects is what you choose to type into the enquiry form. That is your name, your phone number, your email address, your suburb, whether you already have a referral, when you would prefer to be contacted, and whatever you write in the message field. The form software also records the IP address, browser and referring page alongside each submission, which is used for security and to reduce spam.
The form does not ask for your medical records, your Medicare number or any identifying documents, and you do not need to include clinical detail in it. A sentence or two about what is happening is enough.
Analytics and tracking
This website does not currently run third-party analytics, advertising or social media tracking scripts. WordPress, the software the site runs on, may set functional cookies necessary for the site to work. If analytics are added in future, this policy will be updated before they are.
Server logs
As with any website, the hosting server records ordinary technical information such as IP addresses, request times and error records. This is used for security and for keeping the site running, not for building a profile of you.
USE AND STORAGE
What happens to what you send
Information submitted through the enquiry form is used to respond to your enquiry and to arrange an appointment. Submissions are stored in this website's database and sent by email to the practice.
It is not sold, and it is not used for marketing unrelated to your enquiry.
Security
The site is served over an encrypted connection. No website or email system can guarantee absolute security, so as a general precaution do not send clinical details, images or identifying documents through a web form if you would not be comfortable with them sitting in an email inbox. Bring them to the appointment instead, or ask the rooms how they would like them sent.
If it is urgent
The practice notes that it cannot guarantee a timely response to emergencies or urgent matters sent through an enquiry form, and asks that urgent concerns be phoned through on (07) 3870 3708 during business hours. Anything that needs emergency assessment should go to 000 or an emergency department.
YOUR RIGHTS
Access, correction and complaints
Under the Privacy Act you may request access to the personal information held about you and ask for it to be corrected if it is wrong, out of date or incomplete.
Requests about your medical records, or about information given to the practice during a consultation, should go to the rooms on (07) 3870 3708, Monday to Friday, 8:30am to 4:30pm.
Requests about information submitted through this website can be made through the same contact details.
If you are not satisfied
If a privacy concern is not resolved to your satisfaction, you can make a complaint to the Office of the Australian Information Commissioner, which oversees the Privacy Act and the Australian Privacy Principles.

This policy may be updated from time to time. The version in force is the one published on this page, and the date it was last updated is shown at the top.
