The practice asks patients to arrive 10 to 15 minutes before the appointment time, to complete the registration forms and a pain questionnaire.

An initial consultation requires a current referral. A referral from a general practitioner is valid for twelve months; one from a specialist is valid for three months.

Past and recent X-rays, scans and laboratory results, your Medicare card and health fund card, and a list of the medications you are currently taking.

Telephone the office during business hours and allow at least one day's notice, so the appointment time can be offered to another patient on the waiting list.
